A statement from the Indian Air Force said that a missile was accidentally launched on March 9.  A committee of inquiry (court of inquiry) was formed in this incident.

The investigation showed that this incident happened due to mistake of three officials.  The three were also held responsible in the initial investigation.  They were quickly fired from their jobs.

After the incident last March, India's Ministry of External Affairs termed it as "regrettable" and blamed it on a technical glitch.  Pakistan said that the missile landed within 100 km of its country.
